These Rules may be cited as the Day Nurseries (Forms) Rules.
2. Form of register to be maintained by local authority under section 5
The form of register to be kept by every local authority under section 5 of the Act shall be in Form 1 in the Schedule.
3. Form application registration of a day nursery under section 6 (1)
The form of application for registration of a day nursery as required by section 6 (1) of the Act shall be in Form 2 in the Schedule.
4. Form of Certificate of Registration issued by a local authority under section 8 (1)
The form of Certificate of Registration to be issued by a local authority as required by section 8 (1) of the Act shall be in Form 3 in the Schedule.
The forms prescribed may be used with such variations as the circumstances of the case require.
